For a lot of people, the first question about therapy is not “will it help?” but “can I afford it?” Cost is one of the most common reasons people put off reaching out. The good news is that in Ontario, therapy is often more affordable than you might expect, because many extended health plans help cover it. Here is a plain-language guide to how it works.
The short answer
Often, yes. Many extended health benefit plans in Ontario cover sessions with a Registered Psychotherapist, sometimes in full and sometimes up to an annual limit. Whether yours does, and how much, depends on your specific plan. There is no single rule that applies to everyone, so the only way to know for sure is to check your own benefits.
Where therapy coverage usually comes from
Most coverage comes through extended health benefits, the kind that often come with a job. You might have it through:
- Your own employer’s group benefits
- A partner’s or parent’s plan
- A student health plan
- A personal extended health policy
- A Health Spending Account (HSA), which lets you put pre-tax dollars toward health costs, including therapy
If you are not sure whether you have any of these, your HR department or your insurer can usually tell you in a few minutes.
What to check on your plan
When you look at your benefits, or call your insurer, these are the things that matter most:
- Does the plan cover “psychotherapy” or a “Registered Psychotherapist”? The exact wording varies between plans.
- What is the annual maximum, for example $500 or $1,500 per year?
- Is there a per-session cap?
- Do you need a doctor’s referral first? Most plans do not, but a few ask for one.
Knowing those four things tells you almost everything you need.
What about a Registered Psychotherapist (Qualifying)?
This one trips people up. A Registered Psychotherapist (Qualifying) is a fully registered member of the College of Registered Psychotherapists of Ontario who is completing their final supervised hours toward full registration, with the same training and the same College standards. Most plans that cover Registered Psychotherapists also cover Qualifying members, but a small number do not. The quickest way to be certain is to ask your insurer directly: “Do you cover a Registered Psychotherapist (Qualifying) in Ontario?”
Direct billing versus receipts
There are two ways coverage usually works in practice. With direct billing, the therapist submits the claim to your insurer for you, so you only pay any portion that is not covered. With receipts, you pay for the session and then submit a receipt to your insurer for reimbursement. Both are simple. What if you do not have coverage?
Plenty of people pay out of pocket, and there are still ways to make therapy more affordable:
- A Registered Psychotherapist (Qualifying) offers care at a lower fee while completing their final hours, with the same training and standards.
- Some practices, including ours, keep a limited number of sliding-scale spots for people for whom cost is a real barrier.
- Many therapists, again including us, offer a free first consultation, so you can start the conversation without committing to anything.
If money is tight, it is always worth asking. A good therapist would rather find a way to help than turn you away over cost.
A quick script for calling your insurer
If you want to know your coverage in about five minutes, call the number on your benefits card and ask:
- “Does my plan cover psychotherapy or a Registered Psychotherapist?”
- “Do you cover a Registered Psychotherapist (Qualifying)?”
- “What is my annual maximum, and is there a per-session limit?”
- “Do I need a referral?”
Write down the answers, and you will know exactly where you stand.
The bottom line
