What a long-term condition can bring with it

Appointments, medication, and test results get the attention. What often goes unspoken is everything else: the grief, the worry, the tiredness of explaining yourself, and the quiet question of who you are now. That part deserves care too.

Grief for the life you planned

A diagnosis can mean mourning plans, energy, and a version of yourself you expected to keep. That grief is real, and it deserves room.

Uncertainty and health worry

Flare-ups, waiting on results, and not knowing what next year looks like can keep your whole system on alert, long after the appointment ends.

Identity, roles, and independence

When work, parenting, or simply getting through the day changes, the question of who you are now can be hard to sit with alone.

Pain, fatigue, and low mood

Living with symptoms every day wears on mood, and low mood makes symptoms harder to carry. Both sides of that are worth attention.

Being believed

Invisible conditions often come with doubt from others, and sometimes from yourself. Feeling dismissed is its own kind of exhausting.

The people around you

Partners, parents, and children carry it too. Sometimes the most useful conversation is the one you have together.

Who this is for

Adults, teens, and families living with an ongoing health condition, whether it was diagnosed last month or twenty years ago, and whether or not anyone else can see it.

Living with a diagnosis

Diabetes, autoimmune conditions such as MS, lupus, or rheumatoid arthritis, heart and lung conditions, inflammatory bowel disease, cancer during or after treatment, long COVID, and conditions still being worked out.

Chronic pain and fatigue

Fibromyalgia, migraine, persistent pain after an injury, ME/CFS, and the many conditions where the body simply has not returned to how it was.

When an injury became long term

Recovery after a car accident, concussion, or brain injury that has stretched on longer than anyone expected. See also our pages on brain injury and concussion and motor vehicle accidents.

Partners, parents, and family

Caring for someone with a long-term condition, or living in a household reshaped by one. Family sessions are available, and our caregiver support page may also be useful.

Common questions

Do you treat the health condition itself?+
No. Medical care stays with your doctor, specialists, and care team. We support the emotional side of living with a condition: the grief, worry, low mood, identity questions, and relationship strain that often come with it. With your permission, we are glad to coordinate with your care team.
I have been managing this for years. Is it too late to start therapy?+
No. Many people come to therapy long after a diagnosis, often when something shifts: a flare-up, a change in treatment, a new role at home or work, or simply being tired of carrying it alone. Wherever you are in it is a fine place to start.
Can therapy help with chronic pain or fatigue?+
Therapy does not replace medical treatment for pain or fatigue. What it offers is support for what tends to travel with them: low mood, frustration, fear of the next flare-up, and the way symptoms reshape daily life. Approaches such as Acceptance and Commitment Therapy are commonly used alongside medical care for persistent pain.
Is it in person or online?+
Both. You can meet in person at the North York office near Leslie and Sheppard, or online securely from anywhere in Ontario. Many people with a chronic condition find online sessions easier on low-energy days, and you can switch between the two.
Can my partner or family come too?+
Yes. A long-term condition changes life for the people around you as well. Family sessions are available, and some people alternate between individual and family sessions depending on what is needed that week.
Can you come to me at home?+
In some situations, yes. Select therapists offer in-home and community visits in North York and nearby areas. These are offered for accessibility reasons, for example when leaving the house is genuinely hard, with limited mobility or energy, or when travel reliably makes symptoms worse. In-home and community visits are arranged individually, so availability and cost depend on where you are. Book a free consultation and we will confirm both for your area.
Do you offer sessions in Farsi?+
Yes. Sepideh Kojori, Registered Psychotherapist (Qualifying), offers sessions in English and Farsi. See our Farsi therapy page for details.
What does it cost?+
The 15-minute consultation is free. Individual sessions are $170 for 50 minutes and family sessions are $200. In-home and community visits are arranged individually, so availability and cost depend on where you are. Direct billing is available with select therapists.
